After you have been admitted to a CIMBA study abroad program, you will
receive a number of pieces of information to help you prepare.
Additionally, there are numerous forms that must be completed and
returned to our office. Downloadable versions of all required and
voluntary forms are listed below. Additionally, three forms are
available to submit on-line. Links to those forms are also provided.

While the Emergency Medical Information form is voluntary, we encourage
all students to complete and return this information. The information
will be kept confidential and shared only as necessary to ensure that
you have a healthy and safe experience in Italy.
